ABSTRACT

Objective To explore the clinical efficacy of intensity-modulated radiotherapy combined with chemotherapy in the treatment of advanced cervical cancer. Methods All 121 patients with advanced cervical cancer (stage Ⅱ B &Ⅲ A & Ⅲ B) selected in our hospital from June 2012 to June 2014, who were treated with combined chemoradiotherapy, were divided into observation group [intensity modulated radiation therapy (IMRT)group)] and control group [3-dimensional conformal radiation therapy (3D-CRT) group].There was no significant difference between two groups in mean age, body mass index, International Federation of Gynecology and Obstetrics (FIGO) clinical stage, pathological type and chemotherapy mode (P > 0.05).The clinical features of two groups were compared. The treatment and follow-up of two groups were recorded. Results The bone marrow suppression [32.2% (19/59) vs 51.6% (32/ 62)], gastrointestinal reaction [42.4% (25/59) vs 62.9% (39/62)], and rectal reaction rate [27.1% (16/59) vs 45.2% (28/62)] of the observation group were significantly less than that of the control group (P < 0.05).The incidences of genitourinary tract injury [18.6% (11/59) vs 24.2% (15/62)], radiation proctitis [23.7% (i4/59) vs 25.8% (16/62)] and radiation cystitis [16.9% (10/59) vs 20.9% (13/ 62)] of two groups had no significant difference (P > 0.05).Two groups were followed up for 3 years, the local control rate of the observation group was significantly higher than that of the control group (86.3% vs 70.1%) (P < 0.05).Conclusions IMRT combined with chemotherapy in the treatment of advanced cer vical cancer can improve the local control rate of tumor, protect the endangered organ, and reduce the side effects of radiotherapy.

ABSTRACT

Objective To explore the efficacy and safety of irinotecan as neoadjuvant chemotherapy (INAC) plus radical surgery (RS) for cervical cancer. Methods According to International Federation of Gynecology and Obstetrics (FIGO),81 cases were divided into Ⅱ B,ⅡA,and Ⅰ B2 groups.According to the tests of UGT1A1 gene polymorphisms,we adjusted the injection dose of irinotecan. The parameters were analyzed, including the efficacy, operation time and bleeding volume, postoperative pathology, survival time, and adverse reactions. The articles on irinotecan or paclitaxel combined with cisplatin for neoadjuvant chemotherapy between 2005 and 2015 were collected, and compared. Results The effective rate of chemotherapy was 81.5% (Ⅰ B2 group:85.7%;Ⅱ A group:83.3%;and ⅡB group:72.2%),operation time was (5.3 ± 1.1) h,and blood loss was (781 ± 361.7) ml.After chemotherapy,37 cases were delayed diarrhea,70 cases were nausea,48 cases were vomiting, and 40 cases were bone marrow suppression. The infiltration rate, operation time, and blood loss on Ⅱ B group was significantly higher than that on Ⅱ A and Ⅰ B2 groups(P < 0.05),and there was no significant difference in the chemotherapy efficiency, invasion depth, lymphatic metastasis, survival time and adverse reactions(P >0.05).Compared to three articles, the total effective rate in this study was higher than that in previous studies, also in Ⅱ A and Ⅱ B group. Conclusions Irinotecan chemotherapy regimens combined with cisplatin is effective and well tolerated. It is worthy of popularization and application.Detection of UGT1A1 gene polymorphism has guiding significance for chemotherapy regimen on irinotecan combined with cisplatin.

ABSTRACT

Objective To investigate the inhibitory effect of aqueous extract of Taxus chinensis.vat (AETC) combining Cisplatin (DDP) on vitro cultured human lung carcinoma A549 cells, and the effects on resistance genes. Methods The A549 cells were divided into different concentrations of DDP groups, different concentrations of AETC groups, and blank group, and drug effect of 48 h with the method of cell counting kit-8 (CCK-8) and the effect on cell survival were detected.Based on the above results, then A549 cells were divided into DDP combining different concentrations of AETC groups,DDP group, blank control group, and drug effect of 48 h with the method of CCK-8 and the effect on cells survival were detected. The gene expressions of adenosine triphosphate (ATP)-binding cassette subfamily B member 1 trans-porter (ABCB1),ABCG2,and ABCC1 were examined by polymerase chain reaction (RT-PCR).Results Cisplatin 12 μg/ml (DDP),DDP + ATEC 400 μg/ml,DDP + ATEC 800 μg/ml,DDP + ATEC 1 200 μg/ml,DDP + ATEC 1 600 μg/ml,A549 cell inhibition rate of each group was 44.36%,69.61%,74.73%,80.10%,and 74.73%,respectively;Different concentrations of AETC combining DDP could decrease the resistance related gene ABCC1,ABCB1 expressions, and correlated to the dose.AETC combining DDP showed no effects on ABCG2 gene expression. Conclusions AETC combining DDP could inhibit the growth of A549 cells, and decrease the resistance-related gene ABCC1,ABCB1 expressions.

ABSTRACT

Objective To investigate the clinical effect of cisplatin and nedaplatin through comparing the effects of synchronous radiotherapy and chemotherapy of cisplatin and nedaplatin on patients with locally advanced cervical cancers. Methods Clinical data of 54 cases of patients with locally advanced cervical cancers received treatment at our hospital from 2009 to 2011 were analyzed retrospectively.Patients were divided into two groups according to the treatment:group DDP (cisplatin) and group NDP (nedaplatin). The general information,short-term effect,long-term curative effect, and adverse reactions were compared between two groups. Results The general information including gender, age, body mass index (BMI),stages, and pathology had no statistically significant difference between two groups. The response rate (RR)of patients in each group was 100%.The complete remission (CR) rate between groups NDP and DDP had no statistically significant difference.Among the patients ≤60 years old,the CR rate, the survival rate of 1,2,and 3 years, and the median progression-free survival (PFS) had no statistically significant difference.Among the patients > 60 years old,the survival rate of 1,2,and 3 years, and the median PFS had no statistically significant difference, but the CR rate had statistically significant difference. In the non-hematological toxicity, the incidence rate of nausea and vomiting, the effect on renal function in group NDP were obviously lower than those in group DDP.There was no significant difference in liver function damage. In the hematological toxicity, there was no statistically significant difference in the incidence rate of anemia and leukopenia. The incidence rate of decrease of platelet in group NDP was higher than that in group DDP,but it was mainly reflected in the Ⅰ and the Ⅱ level. Conclusions Cisplatin had the same efficacy of concurrent chemoradiotherapy with nedaplatin for the treatment of locally advanced cervical carcinoma with a worse tolerance.
